As a Mississippi adult you can participate in the National Diabetes Prevention Program (DPP) and if you are a Medicare Part B recipient, you may be eligible to enroll in the Medicare DPP benefit for a Lifestyle Coach to help you diet, exercise, reduce stress, lose weight, and other healthy lifestyle changes.

Attend local sessions in your community. Enjoy the in-person company and support of your friends and others who are in the program. A Lifestyle Coach is available in your zip code to help you enroll and support your healthy life pathway to feeling good, looking better, and living longer.

CHECK MY ELIGIBILITY

To get started, please forward your information in the form below if you have been diagnosed with prediabetes to participate in the National DPP. If you are a Medicare Part B recipient, we can simply check your eligibility name and Medicare number in the Medicare Part B database to see if you can participate in the Medicare DPP. Please fill out the form with your name as it appears on your Medicare Health Insurance card along with your Medicare Number.

Medicare requires that participants have a paper copy of their lab results in the ranges below.

The blood test must have been taken within the last year:

Hemoglobin A1C 5.7%-6.4%
Fasting Plasma Glucose (FPG) 110-125 mg/dL
Oral Glucose Tolerance Test (OGTT) 140-199 mg/DL

Schedule an appointment with your Lifestyle Coach for a blood test if you have not had results in the last year.
